What is My AUS Chart?
My AUS Chart is an online, secure health information management application that connects you to portions of their health records, including:
  • Your reminders
  • Past visit records
  • Medications
  • Reviewing demographic information to ensure accuracy
  • Lab results
  • Pharmacy information
I have a medical emergency, can I use My AUS Chart to notify my clinic?
  • No. You should never use My AUS Chart for medical emergencies. Please dial 911 if you have an urgent medical
    These Terms & Conditions describe how information communicated via the Internet is handled and outlines the practices and respect for your privacy. We reserve the right to revoke access at any time for any reason.
    Response to Electronic Communication – AUS will make best effort to provide a timely response to electronic inquiries. In some cases, the clinic staff that needs to respond to an electronic inquiry or other communication may not be immediately available so you should allow at least three (3) business days for a response. Never use My AUS Chart in an emergency situation. If you have an emergency, call 911 immediately.
    We are only able to respond to electronic communications based on the information you give us. If there is insufficient information given, we will be unable to provide accurate and reliable services. Use My AUS Chart messages for care related inquiries only, any misuse of the messaging system (including the use of profanity or inappropriate material) may result in your account being closed.
    Eligibility – Currently My AUS Chart is available to individuals of age 18 or older. Since special rules and provisions are required for minors this service will be made available at a later time.
    E-mail Privacy - Users of My AUS Chart should be aware that they will be notified via email when there is new medical information to be viewed on My AUS Chart. This means that any person with access to your email will be able to see this notification. Although no private medical information will be sent, the notification that new medical information is available by accessing My AUS Chart may be information that you would not want others to know. Users should take this into account when providing an e-mail address.
    Security and Confidentiality - We afford the same degree of confidentiality to medical information stored on My AUS Chart as is given to medical information stored by AUS in any other way. We are committed to protecting the confidentiality of your medical information. Employees’ access and ability to enter or view information is based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. We will identify the records released and note the time and date of access each time you access My AUS Chart. We have taken steps to make all information received online as secure as possible against unauthorized access and use. We are not responsible if you share with others your medical information that you print from My AUS Chart. This information may include visit summary and other relevant data provided to your electronic chart by our technology vendors.
    Opt-out - You have the right to opt-out of the use of the Patient Portal, if so please let your clinic staff know. Please note that if you do opt out, you may not recieve any data associated with Patient Portal, like labs and results, visit notes, or reminders.
    User names and passwords provide two layers of authentication and are stored in an encrypted format. As a My AUS Chart user, your role in maintaining the security of your medical information is:
        1)  Changing your password on a regular basis, and
        2)  Keeping your login ID and password confidential.
    We make every effort to assure that My AUS Chart is always working properly, but there may be times when the system is shut down for maintenance or other purposes. We are not liable for any problems that may occur from being unable to access My AUS Chart. These terms and conditions are subject to change without notice. When they change, you will be able to see the changes when you log-in to My AUS Chart. Your access of My AUS Chart web site shall be deemed as your conclusive acceptance of the terms and condition governing this web site.
What equipment do I need to use My AUS Chart?
  • You will need access to a computer with an internet connection and an email account.
How do I sign up for My AUS Chart?
  • Click on the "Sign Up Now" link under the log-in section. This will bring you to the new member sign up page.
  • To create a My AUS Chart account you will need:
  • Your My AUS Chart access code (the e-mail address you have provided to the clinic)
  • Social Security number
  • Date of Birth
  • Enter the above information into the appropriate fields and click "Next" to continue.
  • Choose a username, password and security question with an answer.
    Note: your My AUS Chart ID can be unique or you can continue to use your e-mail address.
    Click "Next."
  • Type in your username and password and click "Next." You've now successfully logged-in!
Can I contact my physician through My AUS Chart?
  • It is not recommended that you send any confidential e-mails through My AUS Chart. You can however use the, "Contact Clinic" within the portal. This should be used for asking general questions only and not to submit any private data about yourself.
Can my clinic contact me through My AUS Chart?
  • Yes. Your clinic can send you physician selected lab reports, visit notes and reminders.
Is there any charge to use My AUS Chart?
  • No. My AUS Chart is a complimentary service provided by your clinic.
I can't view documents that my clinic has sent me, what do I do?
  • If you cannot open documents in the Visit Notes, Labs, or Reminders section of your My AUS Chart, it is most likely because you do not have Adobe Reader installed.

I'm having troubles signing up, what do I do?
  • Please send an email to patientportal@inpracsys.com with an explanation of the problems you're having and contact information and somebody will get back to you in a timely manner.
